Please Type YES or NO Waivers • Photo/Video Release I hereby assign and grant to Pawsible K9 Dog Training and Hayley May the right and permission to use, reproduce, distribute, and publish the photograph(s), film(s), videotape(s), audio and video recording(s), electronic representation(s) and/or sound recording(s) made of me and/or my dog(s) at any time during the training and/or boarding of my dog(s) by Pawsible K9 Dog Training, and I hereby release Pawsible K9 Dog Training and Hayley May from any and all liability resulting from such use, reproduction, distribution, and publication. Please sign your name and date indicating that you have read these terms and agree to the release agreement: I undersigned hereby acknowledge that I am voluntarily participating in Dog Training/Boarding with Pawsible K9 Dog Training. Pawsible K9 Dog Training will exercise all necessary and proper care and precautions to keep all participant and dogs safe during training sessions/Boarding in order to prevent injury to any person, property, other dog or person. However, because dogs are animals and may be unpredictable, Pawsible K9 Dog Training cannot protect against every potential injury, risk or accident. This Release and Waiver are intended to shift the risk of participating in dog training to the Participant/Owner/Guardian of the dog participating in training/Boarding. I acknowledge that I have had full opportunity to discuss any and all concerns I have regarding foregoing risks Pawsible K9 Dog Training and its authorized representatives. Participant agrees to hold harmless Pawsible K9 Dog Training(Hayley May) free from any and all claims of injury, loss, cost, expenses, or damage caused by the actions of Participant's dog during the course of training/boarding, or under the care, custody or control of Pawsible dog Training. I hereby accept and assume, without reservation, all risks associated with my participation in the training program, including, but not limited to: the risks of any and all injuries to myself, my dog and any of my family members or third parties who may attend; the risks that my dog may cause injury to other persons and/or dogs involved; and the risks that saliva, water, food, and/or other debris may be present in the training area.
